1) A first report of injury need to be made to your employer by you.
2) We need your employer’s name, address and phone number.
3) We need authorization from your employer or their workers’ compensation insurance carrier to treat you.
4) If we are unable to obtain authorization, you may make payment arrangements with our office so we may continue to treat you if you choose.
Automobile Accidents
1) We need the insurance information of all parties involved.
2) The accident needs to be reported to the insurance company of the responsible party. (The person at fault).
3) A claim number is needed of the insurance company at fault.
4) Attorney information if applicable.
Other Personal Injury Cases
1) The insurance information is needed of the responsible party.
2) We also need the responsible party’s name and address.
3) The injury needs to be reported to the insurance company.
4) A claim number is needed.
